Why Does Penile Skin Itch?

1. Scrotal Eczema

Scrotal eczema is one of the most common causes of scrotal itching. In addition to moisture, it often presents with pruritus (itching), erythema (redness), and swelling. It frequently affects individuals with suboptimal hygiene practices—especially those who fail to clean the area regularly—leading to persistent dampness of the penile skin and increased secretions. Concurrently, an unpleasant odor may develop; repeated irritation can result in small papular rashes and intense itching. Initial management should focus on local hygiene and thorough cleansing, while striving to maintain dryness in the affected area—symptoms typically improve gradually with these measures.

2. Prostatitis

Prolonged sitting and working in excessively hot environments are strongly associated with prostatitis. The scrotal skin contains numerous folds and sweat glands; under certain conditions—such as heat exposure or immobility—these glands may become hyperactive, resulting in excessive sweating and sebum production, thereby causing a persistently moist, sticky sensation in the scrotum. Additionally, varicocele (dilated veins in the spermatic cord) may impair local blood circulation beneath the foreskin, leading to elevated scrotal temperature and increased moisture—both contributing to scrotal dampness and pruritus.

3. Physiological Factors

Living or working in a humid environment—or experiencing hot, sweaty weather—can directly irritate the scrotum, triggering dampness and itching. Furthermore, habitual consumption of greasy or fried foods may promote internal “damp-heat” accumulation according to Traditional Chinese Medicine principles; when this damp-heat invades the scrotal region, it manifests as scrotal dampness and pruritus. Similarly, wearing overly tight underwear or experiencing friction from foreign objects may also induce scrotal dampness and itching.
