1. Acute Epididymitis

As the epididymis is in an inflamed state—characterized by congestion and edema—penile engorgement during intercourse may exacerbate swelling and impede resolution of inflammation. If left untreated, epididymitis can impair epididymal function, alter the intraluminal environment, disrupt sperm maturation, reduce sperm fertilizing capacity, and even cause epididymal duct obstruction, thereby impairing sperm transport and potentially leading to infertility.
2. Superficial Balanitis
Non-infectious factors—including local trauma, friction, contraceptive use, soaps, and detergents—may provoke penile itching. Clinically, affected areas may exhibit edematous erythema, erosion, exudation, or even bleeding. Secondary bacterial infection may lead to suppuration and ulcer formation. Patients with penile itching may experience significant pain and impaired mobility. This clinical presentation is termed acute superficial balanitis.
3. Scrotal Eczema
Scrotal eczema is one of the most common causes of scrotal itching and desquamation. In addition to itching and flaking, it often presents with moisture, erythema, and swelling. If these symptoms occur, prompt medical evaluation and treatment at a hospital are recommended to prevent further harm to health. Moreover, environmental factors—such as high humidity, hot weather, and excessive sweating—can also irritate the scrotum, resulting in dampness and pruritus.
