Causes of Scrotal Itching

1. External Irritants

Environmental factors—such as living or working in a humid environment, high ambient humidity, or hot weather leading to excessive sweating—can irritate the scrotal skin and result in moisture accumulation and pruritus. Additionally, a diet rich in greasy or fried foods may promote internal damp-heat accumulation; when this damp-heat affects the scrotum, it can trigger symptoms including moisture and itching. Similarly, wearing overly tight underwear or experiencing friction from foreign objects may also provoke scrotal itching.

2. Infectious Causes

Candidiasis (a fungal infection caused by Candida species) is a common infectious cause of scrotal pruritus and typically presents with intense, severe itching. Patients often describe an unbearable, persistent itch. For symptomatic relief, topical calamine lotion or corticosteroid-containing creams (e.g., Pianping ointment) may be applied to the affected area. Antifungal agents such as miconazole nitrate cream or clotrimazole cream may also be used. However, these medications must be used strictly under medical supervision.

3. Scrotal Eczema

Scrotal itching may also stem from scrotal eczema—a prominent and frequent cause of this symptom. In addition to pruritus and scaling, scrotal eczema commonly manifests with moisture, erythema (redness), and edema (swelling). If these signs appear, prompt medical evaluation at a healthcare facility is strongly recommended to prevent further complications. Furthermore, avoid using excessively hot water for washing, refrain from applying irritating topical agents, eliminate spicy foods from your diet, and minimize scratching.
