What Causes White-Colored Urine?

1. Urethritis

White or cloudy urine may occur in patients with urethritis, cystitis, or—in men—prostatitis. The urine may appear turbid, containing flocculent or feather-like precipitates, and may be accompanied by urinary frequency, urgency, or the discharge of white, viscous material at the beginning or end of urination. In such cases, diagnostic evaluation should include routine urinalysis, urine culture, and—especially for male patients—prostatic fluid analysis.

2. Chyluria

Chyluria occurs when chyle (lymphatic fluid rich in triglycerides) abnormally enters the urinary tract, causing urine to appear milky-white—resembling milk in appearance. This condition may arise from obstruction of the thoracic duct or local lymphatic inflammation, leading to altered lymphatic dynamics and subsequent leakage of lymph into the urinary system. Another major cause is filariasis: parasitic worms invade lymphatic vessels, damaging the lymphatic system and resulting in chyluria.

3. Crystalluria

Crystalluria refers to the presence of excess solutes in urine—such as calcium, uric acid, or phosphates—that exceed saturation levels and precipitate as crystals. This results in cloudy urine, sometimes mixed with visible white crystalline particles. Typically, no urinary irritation symptoms (e.g., dysuria, frequency, or urgency) are present. When a urine sample is left undisturbed, it often separates into layers: a clear upper layer (urine) and a lower layer of whitish crystalline sediment. Further diagnostic workup—including urinalysis and serum uric acid measurement—is required to confirm the diagnosis and differentiate it from other conditions.
