Troponin testing does not require fasting. Cardiac troponin is the primary biomarker for diagnosing myocardial injury and necrosis. Serial troponin measurements, combined with serial electrocardiograms (ECGs), are used to diagnose acute myocardial infarction (AMI) and to perform risk stratification in patients with unstable angina. The most common cause of elevated troponin levels is acute myocardial infarction; other causes include viral myocarditis and unstable angina.

Troponin is a regulatory protein involved in muscle tissue contraction, located on the thin filaments of contractile proteins, where it plays a critical role in regulating both muscle contraction and relaxation. So, is fasting required before a troponin test? Below, we address this question.

Fasting is not required before a troponin test. Troponin is the primary biomarker for diagnosing myocardial injury or necrosis. Serial troponin measurements, combined with serial electrocardiograms (ECGs), are used to diagnose acute myocardial infarction (AMI) and to perform risk stratification in patients with unstable angina. Elevated troponin levels are most commonly associated with acute myocardial infarction but may also occur in viral myocarditis, unstable angina, and heart failure.

Generally, the extent of myocardial necrosis correlates with the magnitude of troponin elevation. In acute myocardial infarction, troponin levels begin to rise significantly within 1–2 hours, peak at 10–24 hours, and remain elevated for approximately 10–15 days for troponin T and 7–10 days for troponin I. Transient myocardial injury—and thus transient troponin elevation—may also occur following intense physical exertion or acute pulmonary embolism; however, levels typically return to normal within 1–2 days. In contrast, troponin elevation secondary to heart failure tends to be chronic, without dynamic changes over time or a distinct peak.
