Introduction
“Dilation and curettage” (D&C) generally refers to a surgical procedure involving the removal of tissue from the uterine cavity. The development of ovarian cysts following D&C may result from postoperative elevation of gonadal hormone levels or intrauterine infection. D&C is commonly performed after medical abortion or surgical abortion to remove retained products of conception or thickened endometrium remaining in the uterus. During the procedure, instruments are used to clear residual tissue from the uterine cavity or to obtain endometrial specimens for pathological examination.

Following a dilation and curettage (D&C) procedure, once an ovarian cyst is identified through appropriate diagnostic examinations, initial management should focus on supportive care and rest to allow the body to recover adequately. Only after baseline recovery has been achieved should surgery for cyst removal be scheduled at an appropriate time. Effective removal of the cystic tissue is essential to restore normal ovarian function.

Why Do Ovarian Cysts Develop After D&C?

“Qinggong” generally refers to a D&C procedure. Post-D&C ovarian cyst formation may result from elevated gonadotropin levels or intrauterine infection.

D&C is commonly performed following medical abortion or surgical abortion to remove retained products of conception or abnormally thickened endometrium from the uterine cavity—either using instruments or by obtaining endometrial tissue for pathological examination. In many pregnant women, elevated gonadotropin levels stimulate changes in granulosa cells of ovarian follicles, leading to transient ovarian enlargement—a physiological phenomenon that typically resolves spontaneously within two to three months post-abortion and usually requires no specific intervention. However, inadequate postoperative care after D&C may lead to intrauterine infection, which in turn can trigger ovarian cyst formation. In cases of severe symptoms, ovarian cystectomy may be indicated. Following surgery, meticulous personal care is essential, including adequate rest and avoidance of excessive physical exertion.

Ovarian cysts detected after D&C should not be surgically removed immediately. Instead, patients require approximately six weeks of convalescence to allow the uterine cavity and overall health to recover sufficiently. Subsequently, surgical excision may be planned based on individual clinical circumstances and comprehensive evaluation of multiple relevant parameters—ensuring optimal restoration of normal ovarian function.
