Introduction

Pelvic inflammatory disease (PID) refers to inflammation affecting the female reproductive organs, the connective tissues surrounding the uterus, and the pelvic peritoneum. Chronic PID often develops from incomplete or inadequate treatment of acute PID, resulting in prolonged disease duration and a stubborn, recurrent course. So, what are the symptoms of chronic PID? Symptoms of Chronic Pelvic Inflammatory Disease

1. Lower abdominal heaviness and pain

A sensation of heaviness and pain in the lower abdomen is one of the most common symptoms of chronic PID. This results from scar formation, adhesions, and pelvic congestion caused by chronic inflammation, leading to discomfort in the lower abdomen. Severe cases may also involve pain in the lumbosacral region. Pain typically intensifies after physical exertion, prolonged standing, sexual intercourse, or around menstruation.



2. Gynecological disorders

Certain gynecological abnormalities are also typical manifestations of chronic PID, including menstrual irregularities, increased vaginal discharge, dysmenorrhea, irregular vaginal bleeding, or menorrhagia—these arise due to irritation of the endometrium. Abdominal pain may also be present during the early stages of PID.

3. Infertility

Infertility is another potential consequence of chronic PID. Without prompt or adequate treatment, chronic PID can lead to tubal obstruction, significantly increasing the risk of infertility or ectopic pregnancy, thereby severely compromising the patient’s health.



4. Systemic symptoms

Patients with PID may experience systemic manifestations such as low-grade fever and persistent fatigue. Prolonged illness may further lead to neurotic symptoms—including insomnia, vivid dreams, poor mental focus, or generalized malaise. Individuals with compromised physical constitution are at higher risk for complications or acute exacerbations of the disease.
