Introduction
Causes of Thrombocytopenia: 1. Drug-induced—e.g., aspirin and other medications may lead to decreased platelet counts. 2. Excessive platelet destruction—primarily classified as either congenital or acquired. 3. Infection-related—e.g., hepatitis viruses, including hepatitis B and C viruses. 4. Hematologic disorders—e.g., myelodysplastic syndromes and other diseases affecting hematopoiesis can cause thrombocytopenia.

Platelets are small cytoplasmic fragments shed from the cytoplasm of mature megakaryocytes in the bone marrow. Although megakaryocytes constitute the smallest proportion—only 0.05%—of nucleated cells in the bone marrow, the platelets they produce play a critically important role in hemostasis. So, what are the causes of thrombocytopenia? The following section addresses this question.

Causes of Thrombocytopenia

1. Drug-induced thrombocytopenia

Thrombocytopenia may result from medication use. Certain drugs can trigger thrombocytopenia. Common examples include aspirin and other nonsteroidal anti-inflammatory drugs (NSAIDs), analgesics, and antithyroid medications used by patients with hyperthyroidism. Therefore, it is advisable to take medications only under the guidance of a physician.

2. Excessive platelet destruction

Thrombocytopenia may be associated with excessive platelet destruction. Platelet destruction is broadly categorized as either congenital or acquired. Acquired excessive platelet destruction further subdivides into immune-mediated and non-immune-mediated forms. Congenital thrombocytopenia tends to be more challenging to treat, whereas acquired forms often respond well to appropriate therapeutic interventions.

3. Infection-related thrombocytopenia

Thrombocytopenia may arise secondary to infection. Viral infections commonly implicated include hepatitis viruses (e.g., hepatitis B and C viruses), Epstein-Barr virus (EBV), and cytomegalovirus (CMV). Additionally, genetic and environmental factors—including prolonged exposure to radiation or chemotherapy—can also lead to thrombocytopenia.

4. Hematologic disorders

Thrombocytopenia may occur in association with hematologic diseases such as myelodysplastic syndromes (MDS) and aplastic anemia. Furthermore, malignancies may infiltrate the bone marrow, thereby impairing platelet production. Autoimmune rheumatic diseases—such as systemic lupus erythematosus (SLE)—may also cause thrombocytopenia due to antigen-antibody reactions that destroy platelets.
