The primary cause of ligament strain is heavy physical activity involving weight-bearing, or sudden changes in body position, which subject muscles, ligaments, fascia, and other soft tissues to external traction forces—resulting in sprains, injuries, or even tears of the muscle and ligament fibers. Symptoms of Ankle Ligament Strain

The classic features of ligament strain include pain and swelling around the ankle joint, with localized pain intensifying during ankle movement. Bruising (ecchymosis) appears on the skin and subcutaneous tissue, accompanied by marked tenderness. Ankle joint mobility becomes restricted, and there is distinct tenderness at the injury site. Passive movement of the ankle joint can provoke pain at the injured area. In severe cases, joint instability and abnormal movement may occur.

When an ankle ligament strain occurs, it is essential to promptly visit a hospital for an X-ray to rule out fractures. If the injury is a simple ligament sprain, conservative management is typically appropriate: rest for 1–2 weeks, avoiding prolonged walking and strenuous activity. Topical application of Yunnan Baiyao aerosol may be used, along with oral administration of blood-activating and analgesic capsules. Alternatively, the affected limb may be immobilized with a cast or brace for approximately 3–4 weeks. Concurrent physical therapy and oral administration of traditional Chinese medicine (TCM) formulations that promote blood circulation and resolve stasis may also be beneficial. For severe ligament injuries—such as complete ligament tears causing ankle joint instability—surgical repair or reconstruction of the ligament may be required.
