What are the hazards of having an overly long foreskin?
Some men experience phimosis (excessively long foreskin). If left untreated, it may adversely affect health, potentially causing penile injury, premature ejaculation, infections, and infertility.
① Penile Injury
With phimosis, adhesions between the foreskin and glans penis commonly occur. Since the glans remains enclosed within the foreskin, its normal development may be impaired due to restricted growth, potentially resulting in a significantly reduced coronal circumference of the glans—thereby compromising sexual intercourse.
② Premature Ejaculation
In cases of phimosis, the glans remains constantly covered by the foreskin, leading to reduced tactile stimulation and friction. Consequently, during sexual intercourse, the glans becomes hypersensitive, increasing the likelihood of premature ejaculation.

③ Infections
Phimosis creates a warm, moist environment conducive to bacterial proliferation. Over time, this may lead to accumulation of smegma, predisposing individuals to balanoposthitis or urinary tract infections. Inadequate management may further trigger other inflammatory conditions, such as prostatitis or orchitis.
④ Infertility
Phimosis may impair male fertility. Poor hygiene or delayed circumcision allows smegma accumulation, which can contribute to prostatitis—thereby reducing sperm motility and altering seminal viscosity, ultimately leading to infertility.
