A dull, intermittent pain in the testicle may be caused by testicular torsion, a common emergency condition of the scrotum. This usually occurs after strenuous activity or trauma to the testicle within several hours prior to onset. Prostatitis is the most common cause of testicular pain; the pain it causes can be widespread and may radiate to the lower abdomen, perineum, around the anus, and inner thighs, or may be limited solely to the testicles. Dull pain can also occur during inflammatory conditions such as orchitis or epididymitis, and some patients may experience severe pain.
