Bilateral sacroiliitis is usually accompanied by pain and morning stiffness. Initially, the pain may be mild and does not affect walking or physical activity, although it might worsen after activity and ease with rest. As the condition progresses, the muscles surrounding the joints become damaged during sleep, reducing the joint's protective function. Poor joint surface congruence, muscle spasms and contractions, joint capsule contraction, and bone spurs may lead to mechanical limitations and functional impairment.
