How to manage IgA nephropathy
Disease description:
Recently, I have been feeling unwell and went to the hospital for a checkup, where I was diagnosed with IgA nephropathy. May I ask, what should I do now?
For IgA nephropathy, treatment strategies should be individualized and mainly include the following aspects:
1. General management: Prevent infections, maintain a low-salt diet, quit smoking, avoid excessive fatigue, and regularly monitor urinary protein and renal function.
2. Pharmacological therapy: Antihypertensive medications and agents to control proteinuria, such as captopril and benazepril, are used for patients with urinary protein excretion >0.5g/day to slow the progression of renal dysfunction. Glucocorticoids, such as prednisone acetate, may be considered if urinary protein remains >1g/day after 3–6 months of initial therapy; however, potential side effects must be carefully considered.
3. Special treatments: For patients with refractory hematuria, tonsillectomy may be considered to reduce antigenic stimulation.
4. Follow-up and management: IgA nephropathy requires lifelong follow-up, as the disease course may change over time. Regular follow-up examinations and adjustments to treatment plans should be conducted according to medical advice.
