There are differing opinions regarding the presence of itching in the rash associated with Henoch-Schönlein purpura (HSP). Most cases of HSP rash are non-pruritic or at most, only mildly itchy. The rash primarily appears on the lower extremities, although it can also occur on the buttocks and upper limbs; involvement of the face and trunk is relatively rare. In the early stages, the rash may resemble small hives or pink maculopapular eruptions that blanch with pressure. It then gradually darkens into red plaques, with petechiae developing in the center, ultimately evolving into purpuric lesions of a dark purple color. Some viewpoints suggest that because HSP is an allergic disorder, the resulting rash may be accompanied by mild itching. However, this occurrence is not common, and any itching that does occur tends to be mild.
