The treatment of anal fistula mainly includes two categories: medical treatment and surgical treatment. In terms of medical treatment, patients can use potassium permanganate solution for sitz bath and apply medications such as Jinhuang ointment externally to relieve symptoms. Additionally, antibiotics may be used during the acute infection phase to combat infection. However, medical treatment usually can only control symptoms and is rarely able to cure anal fistula completely.
Surgical treatment is the primary method for treating anal fistula, including procedures such as fistulotomy, seton placement, and fistulectomy. Fistulotomy is suitable for low-position anal fistulas, involving opening the fistula tract and allowing the wound to heal through granulation tissue growth. Seton placement uses the mechanical pressure of rubber bands or medicinal threads to slowly divide the fistula, offering the advantage of avoiding fecal incontinence. Fistulectomy involves opening the fistula and completely removing the fistula wall until healthy tissue is reached, and it is suitable for simple, low-position anal fistulas.
