Jugular venous distension refers to a phenomenon where the external jugular vein remains excessively filled when the patient is in a semi-recumbent position at an angle of 30°–45°. This phenomenon directly reflects increased right atrial pressure and volume changes, and is commonly seen in patients with right-sided heart failure.

Causes of jugular venous distension include congestive heart failure, which weakens the heart's pumping function and leads to impaired venous return; superior vena cava obstruction syndrome, which blocks the pathway of venous return; cirrhosis, which affects hepatic blood circulation; pulmonary hypertension, which increases the workload on the right heart; and tricuspid regurgitation, which causes backflow of blood. These conditions can all impede venous return, thereby causing jugular vein dilation.

The clinical manifestations of jugular venous distension mainly include visible engorgement, fullness, and swelling of the jugular veins, and sometimes even pulsation may be observed. Treatment should target the underlying cause responsible for the jugular venous distension. For example, diuretics can be used to reduce edema, oxygen therapy can improve hypoxia, postural drainage can help remove secretions, and anti-heart failure medications can enhance cardiac function.
