Right lower abdominal pain accompanied by the sensation of needing to defecate may indicate various conditions and is not always normal. This symptom may be associated with multiple diseases of the digestive system, including but not limited to:

1. Appendicitis: Right lower abdominal pain is often associated with various conditions such as liver disease, gallbladder disease, and appendicitis. Although these conditions may affect intestinal function when severe, they usually do not directly cause a sudden urge to defecate. Appendicitis is one of the most common causes, especially when the pain starts around the navel and gradually shifts and localizes to the right lower abdomen.

2. Inflammatory bowel disease: Such as Crohn's disease, ulcerative colitis, chronic enteritis, or gastroenteritis. These diseases can cause intestinal dysfunction and inflammation, leading to abdominal pain and diarrhea.

3. Constipation or diarrhea: Right lower abdominal pain accompanied by a defecation urge can be a normal physiological phenomenon, especially when the intestines are stimulated, such as by certain foods or during bowel preparation for defecation. These common digestive issues can sometimes cause right lower abdominal pain.
