Pain upon anal contraction mainly involves the following conditions:

1. Hemorrhoids: Both internal and external hemorrhoids can cause discomfort or pain in the anal area. The pain is particularly evident when internal hemorrhoids prolapse, in the presence of thrombosed external hemorrhoids, or when hemorrhoids become inflamed.

2. Anal fissure: A small tear in the skin or mucous membrane around the anus, commonly caused by passing hard or large stools. Anal fissures can cause severe pain during and after defecation, especially upon anal contraction.

3. Muscle spasm: Remaining in the same posture for extended periods, such as sitting or standing for long durations, along with factors like constipation, may lead to tension and spasms in the muscles around the anus, resulting in pain. Adjusting posture appropriately, taking warm sitz baths, and improving local blood circulation can help alleviate this type of pain.

4. Rectal prolapse: Partial or complete protrusion of the rectal mucosa outside the anus, often accompanied by discomfort and pain in the anal region, especially after straining during bowel movements or prolonged standing.

5. Anal sphincter spasm: Involuntary and persistent contraction of the anal sphincter muscles, leading to continuous pain in the anal area.
