Generally, patients with oral ulcers are not advised to eat garlic.

Garlic is a spicy and irritating food. Ingredients such as allicin in garlic may directly irritate the oral mucosa, worsening pain and discomfort caused by the ulcer. Since the mucosa is already damaged due to the ulcer, consuming garlic may delay the healing process or even enlarge the wound. In addition to garlic, spicy foods such as chili peppers, Sichuan peppers, ginger, and stimulants like alcohol and coffee should also be avoided. These ingredients may irritate the oral mucosa and tongue during eating, increasing pain, numbness, and other discomforts for patients with oral ulcers, and may even affect the healing of the ulcerated area.

It is recommended to consume soft, bland liquid or semi-liquid foods, such as porridge, noodles, and steamed eggs, to reduce friction on the ulcerated area. Adequate intake of foods rich in B-complex vitamins, vitamin C, and zinc—such as leafy vegetables, fruits, and nuts—can aid mucosal repair. Mild seasonings such as scallions, ginger, and coriander can be used during cooking to enhance flavor. Plant-based ingredients like lemon zest and herbs can be used to improve food aroma without including spicy components.

Once the ulcer improves, patients may consume moderate amounts of cooked garlic to benefit from its antibacterial and anti-inflammatory properties. At the same time, appropriately adjusting the diet structure and following medical advice for drug treatment can help accelerate the healing of ulcers.
