In general, smaller cysts, especially those caused by localized inflammation or mild obstruction, may gradually shrink or even disappear after the inflammation is controlled or the obstruction is relieved. If the cyst is large, has been present for a long time, or is caused by abnormalities in eye structure or tumors, the likelihood of spontaneous healing is very low.

Some small, asymptomatic eye cysts, such as milia or mild eyelid cysts, may resolve naturally. These cysts typically do not affect vision and are not accompanied by pain or other discomfort. In such cases, the body's own immune system and metabolic processes may sometimes gradually absorb the contents of the cyst, leading to its reduction and eventual disappearance.

If the cyst is relatively large—such as larger than a pea—or located inside the eyeball, spontaneous healing is unlikely due to compression of surrounding tissues or limitations imposed by the ocular environment. It may lead to complications such as decreased vision or elevated intraocular pressure, and timely surgical intervention may be required. If the eye cyst is caused by endocrine disorders, genetic factors, or tissue proliferation following trauma, treatment should target the underlying cause—for example, by regulating hormone levels—as simple observation will not provide a definitive cure.
