How to manage hypotension in heart failure

Introduction
When patients with heart failure experience a drop in blood pressure, it is primarily due to reduced cardiac ejection capacity, resulting in insufficient intravascular blood volume and consequently decreased blood pressure. In such cases, patients are advised to receive positive inotropic agents—such as amrinone, milrinone, or dobutamine—as well as cardiotonic drugs—for example, digoxin.

When heart failure occurs, impaired cardiac ejection leads to a constellation of clinical manifestations, including inadequate peripheral tissue perfusion and organ congestion. Clinically, patients present with symptoms such as dyspnea, cough, sputum production, and hemoptysis. As the condition progresses, dyspnea may evolve into paroxysmal nocturnal dyspnea and orthopnea. So, how should hypotension in heart failure be managed? The following section addresses this question.

Management of Hypotension in Heart Failure

Hypotension in heart failure primarily results from reduced cardiac output, leading to diminished intravascular blood volume and consequent blood pressure decline. In such cases, positive inotropic agents—such as amrinone, milrinone, or dobutamine—are recommended. Cardiotonic agents, including digitalis glycosides (e.g., digoxin), may also be used; however, digoxin is indicated specifically for heart failure caused by systolic dysfunction. Additionally, angiotensin-converting enzyme (ACE) inhibitors—such as enalapril, captopril, fosinopril, and perindopril—should be actively administered. Beta-blockers—including atenolol, labetalol, propranolol, and bisoprolol—are also commonly prescribed. With appropriate pharmacotherapy, most patients experience gradual restoration of blood pressure and stabilization of their clinical condition.

Patients with heart failure are advised to adopt a dietary pattern of small, frequent meals and avoid overeating. They should consume potassium-rich foods, such as bananas, mangoes, potatoes, celery, and kelp. Heavy smoking and alcohol consumption can precipitate acute deterioration of heart failure; therefore, patients are strongly encouraged to quit smoking and abstain from alcohol, while simultaneously improving other unhealthy dietary and lifestyle habits.
