In daily life, some gay men pay close attention to the condition of their genital area as an indicator of overall health. It is not uncommon for men to notice differences in the size of their testicles. So, what causes such asymmetry?
Causes of Asymmetrical Testicular Size
If the two testicles differ significantly in size, further medical evaluation is recommended to identify the underlying cause. Normally, both testicles should be relatively similar in volume. Marked asymmetry may indicate conditions such as orchitis (testicular inflammation), where the affected testicle becomes swollen and enlarged compared with the unaffected side, often accompanied by significant pain. Another possible cause is hydrocele—accumulation of fluid within the tunica vaginalis—which causes visible enlargement of the affected testicle. On palpation, a cystic, fluid-filled sensation can typically be felt, usually without prominent pain. Varicocele—dilation and tortuosity of the pampiniform venous plexus—may also lead to apparent enlargement of the affected testicle due to pooling of dilated veins within the scrotum, resulting in noticeable asymmetry between the two sides of the scrotum.

When testicular size asymmetry is observed, it is essential to undergo a scrotal ultrasound examination at a hospital. Ultrasound can accurately assess whether bilateral testicular development is abnormal. A substantial size discrepancy strongly suggests abnormal development—or atrophy—of the smaller testicle. Clinicians should also consider whether testosterone levels have declined gradually or whether prior testicular trauma may have led to atrophy. In cases where one testicle enlarges progressively, malignancy (e.g., testicular tumor) must be ruled out. Ultrasound is highly effective for diagnosing such conditions. If no pathological abnormalities are detected bilaterally, mild asymmetry requires no intervention. However, if ultrasound confirms a testicular tumor, radical orchiectomy is indicated. For isolated unilateral testicular atrophy, conservative observation is generally recommended. If testicular function remains normal, the individual can maintain normal sexual function and fertility.
