What Causes Glans Pain?
Glans pain can arise from several distinct conditions, each requiring specific management. Local infection of the glans—characterized by pain, redness, swelling, rash, ulceration, or purulent discharge—is a common manifestation of balanitis. Pain should also prompt evaluation for urinary tract stones; most patients with ureteral calculi experience referred pain radiating to the penis and glans region. Such patients often present with nausea, vomiting, and hematuria. Additionally, chronic pelvic floor disorders, pelvic inflammatory conditions, or prostatitis may contribute to glans pain, frequently accompanied by symptoms such as urinary frequency and urethral discharge.

A sharp, stabbing pain in the glans may indicate localized congestion, erosion, exudation, or even bleeding of the glans surface. Secondary infection may result in foul-smelling, whitish, purulent discharge. Gonococcal balanitis typically presents with mild pricking pain, erythema, and swelling of the glans; if progression occurs, inflammation may extend along the entire urethra, leading to urinary frequency, urgency, and dysuria. Non-gonococcal urethritis (NGU) may also cause pricking pain in the glans, manifesting as urethral meatal congestion and edema, along with a burning sensation and pricking pain over the glans surface.
