Most patients with mild fatty liver disease exhibit no characteristic or prominent symptoms. If pain does occur in individuals with fatty liver disease, it often indicates a more severe underlying condition—such as fatty liver–induced hepatitis or hepatic injury.

Fatty liver disease is a relatively common disorder of lipid metabolism. It typically does not cause severe symptoms and usually arises from hepatic metabolic dysfunction, leading to excessive fat accumulation within the liver—commonly resulting in hepatic steatosis (fatty change). Inflammation may irritate the liver capsule, thereby eliciting pain. This pain is typically described as a dull, vague ache localized to the right upper quadrant (RUQ) of the abdomen—the hepatic region. In many cases, significant hepatic inflammation leads to hepatomegaly (enlarged liver), causing abdominal distension and discomfort.

If a patient with fatty liver experiences pain, prompt evaluation of liver function is essential to identify any abnormalities. Additionally, clinicians must consider whether the pain stems from other coexisting conditions—such as gallstones or cholecystitis—as these disorders frequently accompany fatty liver disease.
