Acute gout attacks in the toes are typically triggered by elevated serum uric acid levels. After the acute phase subsides and pain resolves, medications that either inhibit uric acid production or promote its excretion—such as febuxostat, allopurinol, or benzbromarone—may be prescribed orally. Surgical intervention—including debridement or orthopedic surgery—may be necessary if tophi erode the overlying skin, fail to heal, cause joint deformity, or progressively enlarge, thereby severely impairing the patient’s quality of life.

Gout affecting the big toe falls under the category of gouty arthritis. Uric acid crystal deposition in the toe joints leads to localized redness, swelling, and pain—often occurring suddenly at night. Patients frequently avoid movement due to intense discomfort, and such episodes are commonly associated with recent consumption of high-purine foods, such as beer and seafood. In daily life, adherence to a low-purine diet is essential: high-purine foods—including seafood, organ meats, and beer—should be limited.
