Introduction
The normal fasting insulin level is 2.6–25 IU/mL. A low fasting insulin level indicates impaired pancreatic islet function and reflects the intrinsic islet function in the absence of exogenous insulin administration. Conversely, an elevated fasting insulin level—without exogenous insulin use—is commonly observed in obese patients with insulin resistance.

Insulin is a hormone secreted by the pancreatic islets and plays a key role in regulating carbohydrate metabolism and maintaining blood glucose concentration. It is the only hormone in the human body capable of lowering blood glucose levels. Insulin deficiency leads to hyperglycemia and may result in various complications. So, what is the normal fasting insulin level?

Normal Fasting Insulin Range

The normal fasting insulin level ranges from 2.6 to 25 µIU/mL. Individual values may vary, and reference ranges may differ slightly among laboratories or hospitals. A fasting insulin level below 2 µIU/mL—especially if it is only a fraction of a unit—suggests impaired pancreatic islet function and reflects the endogenous insulin-secreting capacity in the absence of exogenous insulin administration. Conversely, a fasting insulin level above 25–30 µIU/mL (without exogenous insulin use) is commonly observed in obese individuals with insulin resistance. This condition typically corresponds to hyperinsulinemia and is frequently seen in obese individuals with abnormal glucose metabolism—often representing early-stage impaired glucose tolerance (IGT).

In obesity-related insulin resistance, the body compensates by secreting increased amounts of insulin to counteract dysregulated glucose metabolism. Due to reduced metabolic sensitivity, insulin secretion exceeds baseline levels. Contrary to intuition, higher fasting insulin levels are not desirable; instead, elevated levels typically indicate insulin resistance—particularly in obese individuals. Following weight loss, insulin sensitivity improves and insulin levels often decrease.
