Introduction
In elderly individuals, the diagnostic criteria for diabetes are a fasting blood glucose level ≥ 7.0 mmol/L or a postprandial (2-hour) blood glucose level ≥ 11.1 mmol/L. For patients without typical symptoms of diabetes, if only the fasting blood glucose is ≥ 7.0 mmol/L or only the postprandial blood glucose is ≥ 11.1 mmol/L, repeated blood glucose measurements are required. A diagnosis of diabetes is confirmed when elevated values are consistently observed on multiple occasions.

Regardless of age, individuals must maintain blood glucose levels within the normal range. For older adults, good glycemic control is essential for healthy aging and longevity. So, what constitutes the normal blood glucose range for elderly individuals?

Normal Blood Glucose Range for Elderly Individuals

The diagnostic criteria for diabetes in older adults are: fasting plasma glucose ≥7.0 mmol/L or postprandial (2-hour) glucose ≥11.1 mmol/L. In asymptomatic individuals, if only fasting glucose exceeds 7.0 mmol/L or only postprandial glucose exceeds 11.1 mmol/L, repeated glucose testing is required. A diagnosis of diabetes is confirmed when multiple consecutive measurements exceed these thresholds.

For relatively healthy older adults aged 60–70 years without severe complications, fasting glucose, 2-hour postprandial glucose, and HbA1c levels should all be maintained within the recommended target ranges for elderly individuals. For adults aged 70 years and older with type 2 diabetes—even those in good general health and without cardiovascular or cerebrovascular disease—the target fasting glucose should be 6–7 mmol/L, and 1-hour postprandial glucose should be controlled at 8–9 mmol/L, to keep overall glucose levels within the normal range.

In elderly patients with diabetes who already have cardiovascular or cerebrovascular complications, intensive glycemic control is generally not advisable. Accordingly, glycemic targets are somewhat relaxed: fasting glucose should be maintained at 7–9 mmol/L; 2-hour postprandial glucose at 8–11.1 mmol/L; and HbA1c at 7.0%–7.5%.
