During infection, this GAG layer becomes disrupted, leading to pain localized to the bladder region—i.e., lower abdominal pain.

Lower abdominal pain is a common symptom encountered in daily life and may occur frequently. Some individuals experience lower abdominal pain after urination. So, what causes lower abdominal pain following urination?

Causes of Lower Abdominal Pain After Urination

Lower abdominal pain after urination may stem from several possible causes:

1. Acute or chronic cystitis: When the bladder becomes inflamed, bacterial invasion damages the bladder mucosal epithelium, leading to mucosal congestion, edema, and even erosion. A protective glycosaminoglycan (GAG) layer normally covers the bladder mucosa, preventing direct contact between urine and the mucosa. During infection, this GAG layer is disrupted, allowing urine to directly contact the bladder mucosa and submucosa, resulting in pain localized to the bladder region—i.e., lower abdominal pain.

2. Bladder stones: In patients with bladder stones, mucosal irritation caused by the stones leads to microscopic mucosal injury. Urine contacting these injured areas triggers pain. During urination, bladder stones may float within the urine stream, intensifying mucosal irritation and thereby worsening lower abdominal pain after voiding.

3. Chronic prostatitis: In young and middle-aged men, chronic prostatitis may be responsible. Inflammatory stimulation of the prostate can manifest as lower abdominal pain after urination, often accompanied by perineal fullness or discomfort, and/or urinary hesitancy.

4. Other conditions: Pelvic inflammatory diseases—including endometritis, salpingo-oophoritis, or broader pelvic inflammatory disease—may also cause lower abdominal pain following urination.
