The severity of lumbar transverse process fractures must be assessed based on specific circumstances. Generally, isolated lumbar transverse process fractures are not considered serious, as the transverse processes are accessory structures of the lumbar vertebrae and not primary weight-bearing components, and there are no major nerves in their vicinity. In such cases, patients typically require bed rest, avoidance of strenuous activity, and may use non-steroidal anti-inflammatory drugs (NSAIDs) depending on the level of pain. However, if the transverse process fracture is accompanied by vertebral compression fracture or other injuries, the condition may be more serious. In cases of mild vertebral compression without neurological damage, strict bed rest may be sufficient to allow fracture healing. If the vertebral compression exceeds one-third of the height, or if there is compression of the posterior dural sac or nerve roots, surgical intervention with internal fixation may be required.
