During my liver function test, the doctor mentioned that my total bilirubin level was elevated. I would like to ask the doctor, what are the possible causes of elevated total bilirubin?

Dr. Yang Ziqi

Total bilirubin refers to the total amount of bilirubin in the blood, including both conjugated and unconjugated bilirubin. Elevated total bilirubin levels may be caused by the following conditions:

1. Hemolytic jaundice: Excessive breakdown of red blood cells exceeds the liver's capacity to process bilirubin, leading to increased unconjugated bilirubin. This may be caused by hereditary hemolytic anemia, autoimmune hemolytic disorders, or other diseases.

2. Hepatocellular jaundice: Liver diseases such as hepatitis, cirrhosis, or liver cancer impair the liver's normal metabolism of bilirubin, resulting in elevated levels of both conjugated and unconjugated bilirubin.

3. Obstructive jaundice: Biliary tract issues such as gallstones, tumors, or inflammation block the excretion of bile, preventing bilirubin from being normally excreted into the intestines, thus increasing conjugated bilirubin levels.

4. Neonatal jaundice: In newborns, bilirubin metabolism is not yet fully mature, which may lead to transiently elevated total bilirubin levels.

If total bilirubin levels are elevated, prompt medical attention is recommended.
