Januvia (sitagliptin phosphate tablets), as an oral hypoglycemic agent of the dipeptidyl peptidase-4 (DPP-4) inhibitor class, is primarily used to improve glycemic control in patients with type 2 diabetes.

1. Standard dosage: The generally recommended starting dose is 100 mg once daily, which can be taken either before or after meals, unaffected by food intake.

2. Timing of administration: Patients may choose to take the medication in the morning, at noon, or in the evening, but it is recommended to maintain a consistent time each day to ensure stable drug concentration in the body.

3. Target population: It is primarily indicated for patients with type 2 diabetes whose blood glucose levels remain uncontrolled despite dietary and exercise management.

4. Dosage adjustment: For patients with renal insufficiency, dosage adjustment is required according to renal function status. For patients with mild to moderate renal impairment, specifically when the endogenous creatinine clearance rate is above 50 ml/min, no dosage adjustment is necessary; when the creatinine clearance rate is between 30 and 50 ml/min, the dose should be reduced to 50 mg once daily; for clearance rates below 30 ml/min, a reduction to 25 mg daily is recommended.
