Pediatric keratitis is primarily bacterial or viral in origin. For bacterial keratitis, topical application of 0.3% ofloxacin eye drops, levofloxacin eye drops, or tobramycin eye drops is recommended, followed by cycloplegic agents such as 0.5% or 1.0% atropine sulfate ointment. For viral keratitis, topical antiviral agents are administered.

The eye is not only structurally complex but also highly delicate. As the eye’s protective barrier, the cornea is susceptible to inflammation—such as viral keratitis or acute keratitis—when compromised by bacterial invasion or infection, significantly disrupting the patient’s daily life and work. So, what should be done if a child develops acute keratitis?

Management of Acute Keratitis in Children

In children, keratitis is primarily bacterial or viral in origin. For bacterial keratitis, topical treatment includes 0.3% ofloxacin eye drops, levofloxacin eye drops, or tobramycin eye drops, followed by cycloplegic agents such as 0.5% or 1.0% atropine sulfate ointment. For viral keratitis, topical antiviral agents—such as acyclovir eye drops or ganciclovir eye gel—are indicated. Atropine sulfate ointment may also be used, along with preservative-free artificial tears (e.g., Bright Eyes solution).

Bacterial infection can cause acute keratitis, a condition that progresses rapidly and requires prompt antibiotic therapy—such as topical levofloxacin hydrochloride eye drops or tobramycin eye drops. Viral keratitis, by contrast, tends to recur and necessitates antiviral treatment; options include interferon eye drops and tobramycin eye drops. Fungal keratitis is relatively rare in acute cases but carries high morbidity and mortality; timely antifungal therapy—such as fluconazole eye drops or natamycin eye drops—is essential.
