In general, toothache may be caused by conditions such as pulpitis, apical periodontitis, periodontitis, food impaction, periodontal abscess, or dentin hypersensitivity. Toothache has many potential causes, and prompt relief should be sought once the underlying cause is identified. Pulpitis is a common cause of toothache, typically resulting from deep dental caries. Bacteria invade the pulp chamber, leading to congestion and inflammation of the dental nerve.

  Toothache is a common condition that can be severe and significantly impact overall health—even disrupting daily life. Prompt management and treatment are therefore essential. Since toothache has numerous potential causes, identifying the specific underlying cause is crucial for effective intervention. So, what causes toothache?

  What Causes Toothache?

  In general, toothache may result from conditions such as pulpitis, apical periodontitis, periodontitis, food impaction, periodontal abscess, or dentin hypersensitivity. Given the wide range of possible causes, timely symptom relief is important once the cause is identified. Pulpitis is one of the most common causes of toothache, typically arising from deep dental caries. Bacteria invade the pulp chamber, triggering inflammation and congestion of the dental nerve. Pain is often spontaneous, intensifies at night, and worsens with thermal (hot or cold) stimuli. Apical periodontitis causes persistent pain, usually due to trauma. Affected teeth may feel “loose” or “floating,” impairing normal masticatory function. Patients often cannot precisely identify the affected tooth. Periodontitis presents with pain similar to apical periodontitis; additionally, periodontal pockets may form and teeth may become mobile.

  Gingival tissues may also exhibit recurrent bleeding and swelling—symptoms commonly caused by food impaction between teeth. Particularly when impacted food is very hard or very small, it can trigger localized pain known as “food impaction pain,” which falls under the broader category of dental disorders. Inflammatory changes in the periodontal tissues may progress to suppurative infection, resulting in a painful periodontal abscess. Dentin hypersensitivity manifests as pain triggered by acidic, sweet, spicy, cold, or hot stimuli—but pain subsides promptly once the stimulus is removed.

  It is advisable to avoid spicy foods—even mild peppers—as some patients report toothache triggered by emotional stress such as anger. Black pepper and other pungent spices may exacerbate irritability; thus, they should be avoided or consumed sparingly during episodes of toothache. We hope this information proves helpful.
