Whether the intrauterine device (IUD) must be removed should be determined based on age and individual circumstances. It is advisable to remove the IUD before menopause to avoid complications after menopause, such as uterine atrophy, IUD embedding, abdominal pain, bloating, and other discomforts. After menopause, removal of the IUD may be more difficult due to cervical atrophy and uterine shrinkage, increasing the risk of complications such as IUD embedding or fracture.

Although the two-child policy is now in effect, many women still opt for intrauterine devices (IUDs) as their preferred contraceptive method—primarily due to their high safety profile. But does an IUD always need to be removed?

Must an IUD Always Be Removed?

Whether removal is mandatory depends on a woman’s age and individual clinical circumstances. It is advisable to remove the IUD before menopause to prevent complications such as uterine atrophy, IUD embedment, abdominal pain, or bloating after menopause. Removal post-menopause poses greater challenges: cervical and uterine atrophy may lead to IUD embedment or even fracture, increasing procedural difficulty and risk of adverse outcomes.

An IUD is a long-term contraceptive device placed inside the uterine cavity. Its mechanism of action involves releasing copper ions, which alter the intrauterine environment, induce sterile endometrial inflammation, and interfere with blastocyst implantation—thereby achieving effective, long-lasting contraception. This method generally causes minimal harm to a woman’s body and does not impair future fertility.

After IUD removal, sexual intercourse and tub baths should be avoided. If additional procedures such as endometrial curettage or hysteroscopy were performed, abstinence and avoidance of tub baths should be extended to four weeks. Also monitor for fever or significantly worsening abdominal pain. For women of childbearing age, appropriate contraception must be resumed promptly after IUD removal. If reinsertion of an IUD is not feasible, an alternative, reliable contraceptive method should be selected to prevent unintended pregnancy. We hope this information is helpful.
