Neck stiffness so painful that one cannot turn over or get out of bed
"Wry neck" is relatively common in daily life. It may occur due to improper sleeping posture, causing pain in the neck muscles. Wry neck can also be associated with neck exposure to cold, as detailed below:
When the pillow is too high or sleeping posture is incorrect, it may lead to injury of the neck muscles, resulting in neck pain and discomfort—commonly known as "wry neck." When patients attempt movements such as turning over or rotating the neck, the contraction of affected neck muscles can exacerbate the pain, making them reluctant to move due to discomfort.
For severe cases of wry neck that make lying down difficult, gentle manual massage of the affected neck muscles is generally recommended. This can be combined with local physical therapy using warm, moist towels for heat application. Topical traditional Chinese medicines with blood-activating and stasis-resolving effects—such as safflower oil, orthopedic water, or Yunnan Baiyao spray—can also be applied locally. Local heating methods such as infrared irradiation may be used to enhance subcutaneous penetration of these medications. If pain is significant, topical application of Voltaren gel (diclofenac) can be used, or under a doctor's guidance, oral anti-inflammatory and analgesic drugs such as aceclofenac, diclofenac, or celecoxib may be taken directly. These medications effectively reduce the aseptic inflammation at the affected site, thereby achieving rapid relief from neck pain.
