Introduction
A positive nitrite test may indicate nephritis, or could be caused by urgency, urinary tract infection, cystitis, kidney stones, etc. Treatment should follow medical advice and may include medication. A definitive diagnosis and symptomatic treatment depend on kidney function tests. If the patient experiences symptoms such as urgency or dysuria, white blood cells in the urine routine test may also increase significantly, resulting in a positive test result.

Positive nitrite test results may indicate nephritis, or could be caused by urgency of urination, urinary tract infection, cystitis, kidney stones, and other conditions. Treatment should follow medical advice and may involve medication; however, a definitive diagnosis requires kidney function tests to guide appropriate therapy.

1. Nephritis

Nephritis is usually caused by various viral or bacterial infections, commonly presenting symptoms such as hematuria (blood in urine) and flank pain. A positive nitrite result in routine urinalysis may suggest Escherichia coli infection leading to pyelonephritis, but confirmation requires further evaluation with kidney function tests. Under a doctor's guidance, medications such as ampicillin and probenecid capsules or Fushenning tablets may be used for treatment.

2. Urinary Urgency

If a patient experiences symptoms like urgency and pain during urination, white blood cells in the urine may also significantly increase, resulting in a positive test. In such cases, antibiotics such as cefuroxime axetil tablets or amoxicillin tablets can be prescribed under medical supervision.

3. Urinary Tract Infection (UTI)

Urinary tract infections are typically caused by ascending bacterial infections. Depending on the site of infection, they are classified as upper or lower UTIs. Lower UTIs cause symptoms such as dysuria (painful urination) and urgency, while upper UTIs may present with nausea, headache, and high fever. Nitrites are generally metabolic byproducts of bacteria; thus, a positive nitrite test in urinalysis can help diagnose a urinary tract infection. Under medical guidance, medications such as cefixime tablets or linezolid tablets may be used for treatment.

4. Cystitis

Cystitis is usually caused by bacterial infection. When large numbers of bacteria are present in the urine, they break down urea into nitrites, leading to a positive nitrite test result. Treatment may include oral administration of drugs such as levofloxacin hydrochloride capsules or azithromycin capsules, as directed by a physician.

5. Kidney Stones

Kidney stones form when certain substances in the urine become highly concentrated and aggregate into crystalline particles. This condition often leads to increased white blood cells in the urine, which may result in a positive nitrite test. Under a doctor’s guidance, medications such as diclofenac sodium tablets or indomethacin tablets may be used for symptom management.

It is recommended to undergo comprehensive medical evaluation at a hospital to determine the underlying cause and select an appropriate treatment plan accordingly.
