Patients with Henoch-Schönlein purpura (HSP) should avoid certain foods for at least one month or longer, as the onset of HSP is associated with food allergies, thus requiring a prolonged period of dietary restriction.

HSP commonly affects the lower legs. Its hallmark is palpable purpura, often accompanied by varying degrees of joint pain, abdominal pain, and kidney involvement. The exact cause of this condition remains incompletely understood. Possible triggering factors include infections, food or drug allergies, insect bites, and cold exposure. The skin pathology shows leukocytoclastic vasculitis. HSP can occur at any age but is more common in children. Skin lesions are typically symmetrical and distributed on the extensor surfaces of the limbs, especially the lower extremities and buttocks. Lesions are polymorphic, presenting as palpable petechiae or hemorrhagic maculopapules of varying sizes that do not blanch under pressure. Some lesions may be edematous, resembling urticaria or erythema multiforme. The disease course tends to be protracted.

At onset, patients often experience joint pain, primarily affecting the knees and ankles, sometimes accompanied by joint swelling. During the course of the illness, abdominal pain and gastrointestinal bleeding may develop, occasionally mimicking acute abdomen. Some patients develop kidney involvement manifesting as proteinuria and hematuria; severe cases may progress to renal insufficiency. Since HSP is a form of vasculitis and some cases are linked to food allergies, dietary restrictions should last for at least one month or longer.

In addition to dietary avoidance, patients with HSP should maintain a balanced diet rich in nutrients, including fresh vegetables, fruits, and whole grains. They should avoid overeating and excessive alcohol consumption. It is also important to avoid known allergens and irritants, while maintaining healthy lifestyle habits and good personal hygiene, all of which help prevent and control the occurrence and recurrence of HSP.
