What are the causes of isolated low FT4?

Introduction
Low free T4, also known as low free thyroxine (FT4), is usually caused by physiological factors or pathological conditions such as hypothyroidism and hypoalbuminemia. It commonly occurs due to long-term insufficient iodine intake, abnormalities in the immune system attacking thyroid tissue, or the thyroid gland's inability to secrete thyroid hormones normally.

   In general, isolated low FT4, also known as low free thyroxine (FT4), is usually caused by physiological factors or pathological conditions such as hypothyroidism and hypoalbuminemia. The specific causes are analyzed as follows:
  I. Physiological Causes
  Low FT4 often results from long-term insufficient iodine intake, leading to iodine deficiency in the body and subsequently reduced levels of free thyroxine (FT4). Patients can increase their dietary intake of iodine-rich foods to replenish the required iodine.
  II. Pathological Causes
  1. Hypothyroidism
  This commonly occurs due to abnormalities in the patient's immune system that attack thyroid tissue, impairing the thyroid gland's ability to produce thyroid hormones normally, thus causing low FT4 levels. Patients may be treated under medical guidance with medications such as liotrix tablets or levothyroxine sodium tablets.
  2. Hypoalbuminemia
  Reduced thyroid hormone levels can impair liver function in protein synthesis, leading to hypoalbuminemia. Patients with this condition often also present with low free thyroxine (FT4) levels. Under a doctor’s supervision, treatment may include thyroid tablets or human albumin injection to supplement the body's albumin.
  Besides the above-mentioned causes, other conditions such as Hashimoto's thyroiditis and subacute thyroiditis may also contribute. If the underlying cause is unclear, patients should promptly seek medical evaluation at a hospital to identify the cause and receive timely treatment.
