What causes the eyes to roll upward and show the whites?

Introduction
Rolling of the eyes upward may be related to incomplete maturation of the central nervous system, inward-growing eyelashes (trichiasis), ophthalmoplegia, epilepsy, encephalitis, and other causes. To prevent this phenomenon, it is important to maintain good eye hygiene and ensure adequate sleep. For known medical conditions, follow medical advice for treatment and proper disease management to help reduce episodes of upward rolling of the eyes.
Under normal circumstances, upward rolling of the eyes (exposing the whites) may be caused by factors such as an immature central nervous system, trichiasis, ophthalmoplegia, epilepsy, or encephalitis. General treatment and medication should be administered under medical supervision. Specific analyses are as follows:
1. Immature Central Nervous System
This condition commonly occurs in infants and young children. Due to the incomplete maturation of their central nervous system, coordination between nerves and muscles controlling eye movement may be impaired, leading to upward rolling of the eyes. No special treatment is required; this phenomenon typically resolves gradually as the child grows and the nervous system matures.
2. Trichiasis
Trichiasis refers to abnormally directed eyelashes that turn backward toward the eyeball, irritating the cornea and conjunctiva and causing discomfort. Prolonged irritation may lead to upward rolling of the eyes, accompanied by symptoms such as eye pain, tearing, and a foreign body sensation. Under medical guidance, medications such as tobramycin eye drops, levofloxacin eye drops, and acetaminophen tablets can be used to relieve infection and associated symptoms.
3. Ophthalmoplegia
Ophthalmoplegia involves damage to the nerves or muscles that control eye movement, resulting in impaired eye motility. When ptosis (drooping of the upper eyelid) occurs due to ophthalmoplegia, individuals may unconsciously roll their eyes upward to see clearly ahead. This may also be accompanied by blurred vision, double vision, and other symptoms. Treatment under a physician’s guidance may include medications such as mecobalamin tablets, prednisolone acetate tablets, and amoxicillin capsules.
4. Epilepsy
Epilepsy is a chronic neurological disorder caused by abnormal electrical discharges in the brain. During seizures, patients may experience loss of consciousness, convulsions, and upward rolling of the eyes. Other symptoms may include foaming at the mouth and muscle rigidity. Following a doctor's advice, medications such as carbamazepine tablets, sodium valproate sustained-release tablets, and phenytoin sodium tablets can be used for treatment.
5. Encephalitis
Encephalitis is an inflammatory condition of the brain parenchyma caused by pathogen invasion. It may lead to serious consequences such as increased intracranial pressure and brain tissue damage, which can result in upward rolling of the eyes. In addition to upward eye deviation, symptoms may include headache, fever, nausea, vomiting, and altered consciousness. Patients should follow medical recommendations and use medications such as azithromycin injection, mannitol injection, and ibuprofen sustained-release capsules for treatment.
To prevent or reduce episodes of upward rolling of the eyes, it is important to maintain good eye hygiene and adequate sleep. Individuals diagnosed with conditions such as epilepsy or encephalitis should strictly adhere to their doctor's treatment plans and medication regimens to control disease progression.
