How long can elderly patients with kidney failure survive?
In elderly patients, the prognosis of renal failure primarily depends on the current serum creatinine level and the corresponding stage of chronic kidney disease (CKD)—whether it is Stage I–IV or Stage V. If the condition remains at Stage I–IV without progression to Stage V, it generally has minimal impact on daily life, and life expectancy tends to be relatively long.

If the disease progresses to Stage V, renal replacement therapy—either hemodialysis or peritoneal dialysis—becomes necessary. With regular, well-managed dialysis, prognosis largely depends on associated comorbidities, particularly blood pressure control and cardiac function. Uncontrolled severe hypertension increases the risk of intracerebral hemorrhage, which can be immediately life-threatening. Similarly, significantly impaired cardiac function poses an ongoing risk of acute cardiac events, thereby adversely affecting survival.

Additionally, dietary management is critical: patients must strictly avoid high-potassium foods. Serum potassium levels exceeding 6.5 mmol/L—especially in those with underlying heart disease—significantly increase the risk of sudden cardiac arrest. However, with consistent, appropriate dialysis, most patients can expect a life expectancy of approximately 10 years. Exceptions include patients with diabetic nephropathy, whose median survival is typically around 5 years—shorter than that observed in patients with renal failure secondary to other causes such as chronic glomerulonephritis.
