Introduction
Normal semen is grayish-white or slightly yellowish in color. However, some men ejaculate semen that appears red, reddish-brown, or contains blood streaks—a condition medically termed “hematospermia.” Hematospermia may have various underlying causes; common ones include seminal vesicle calculi and seminal vesiculitis. Additionally, urethral injury cannot be ruled out—such injuries typically result from overly vigorous sexual activity.

Following orgasm during sexual intercourse, men typically experience ejaculation. Under normal circumstances, semen is grayish-white in color; however, some men ejaculate red-tinged semen. Hematospermia—a pathological condition—indicates an underlying health issue. Thus, the presence of blood in semen signals potential abnormalities within the body. What Causes Red-Colored Semen?

Normal semen is typically grayish-white or slightly yellowish. In contrast, some men may ejaculate semen that appears red, reddish-brown, or streaked with blood—clinically termed hematospermia. Hematospermia often goes unnoticed unless bleeding is substantial, at which point patients usually seek medical attention. By then, the condition may already be relatively advanced. Common causes include seminal vesicle calculi and seminal vesiculitis. Additionally, red-tinged semen may also result from urethral injury—particularly anterior urethral trauma—which can occur due to overly vigorous sexual activity. In most such cases, unless bleeding is profuse, spontaneous resolution typically occurs.

Knowledge Extension: How to Manage Hematospermia

1. Dietary Restrictions

Patients experiencing hematospermia should observe certain dietary precautions. For instance, alcohol consumption and spicy foods should be avoided, as they may trigger recurrence. Patients should also avoid “heat-inducing” or “trigger” foods—such as seafood—and foods known to promote internal heat, like scallions.

2. Pharmacological Treatment

Antibiotics are commonly used to treat hematospermia. However, prolonged antibiotic use may lead to antimicrobial resistance. Therefore, prior to initiating antibiotic therapy, a culture and sensitivity test should be performed to identify the most appropriate agent for targeted treatment. Typical regimens include levofloxacin or second- or third-generation cephalosporins, administered continuously for 7–10 days.
