In cases of severe, refractory pain unresponsive to conventional pharmacotherapy, localized nerve block procedures may be considered.
The most fundamental strategy for preventing PHN is timely intervention. Antiviral therapy should be initiated as early as possible—ideally within 48 hours of rash onset, and no later than 72 hours. Early administration of a one-week course of systemic corticosteroids may help rapidly suppress neuritis and thereby reduce the risk of developing PHN. This preventive approach is especially critical in elderly patients or those presenting with severe shingles and intense pain; once PHN develops, treatment becomes significantly more difficult.
