Introduction
In general, the most common symptom of advanced nasopharyngeal carcinoma is cervical lymphadenopathy, resulting from tumor metastasis to the lymph nodes. Approximately 80% of patients develop unilateral or bilateral lymph node involvement; these nodes are typically mobile and painless in the early stages but become fixed and adherent to the skin or deep tissues in later stages. Secondly, a range of ocular symptoms may occur, including decreased visual acuity, visual field defects, proptosis, diplopia, and even blindness.

What Are the Clinical Manifestations of Advanced-Stage Nasopharyngeal Carcinoma?
Generally, the most common symptom of advanced nasopharyngeal carcinoma is cervical lymphadenopathy—enlargement of neck lymph nodes—resulting from tumor metastasis to regional lymph nodes. Approximately 80% of patients develop unilateral or bilateral lymph node involvement. In early stages, these enlarged nodes remain mobile and painless; however, in later stages, they become fixed and adherent to overlying skin or deeper tissues. Additionally, ocular symptoms may occur, including visual impairment, visual field defects, proptosis (bulging eyes), diplopia (double vision), and even blindness. Furthermore, when cranial nerves—particularly the trigeminal nerve—are invaded by the tumor, patients may experience headaches and facial numbness.
In advanced stages, nasopharyngeal carcinoma frequently metastasizes to distant sites—a hallmark feature shared with many other malignancies. Bone metastases are the most common form of distant spread; they predispose patients to pathological fractures and cause persistent pain. Other manifestations of advanced disease include cancer-related cachexia: progressive tumor growth consumes substantial energy and fat stores, leading to systemic debilitation and severe weight loss, thereby threatening life. Nasal obstruction is also a classic symptom of late-stage nasopharyngeal carcinoma. Initially, obstruction is mild when the tumor is small; however, as the tumor enlarges, it progressively obstructs the nasal passages, worsening nasal congestion.
