In children, jaundice is the most common early manifestation of hepatitis B infection; thus, some physicians refer to pediatric hepatitis B infection as “jaundice-type hepatitis.” Clinical signs include yellowish discoloration of the face—particularly the sclera and conjunctiva—as well as generalized yellowing of the skin. Additional indicators may include diarrhea with pale, clay-colored stools or dark-yellow urine resembling strong tea. These manifestations collectively characterize jaundice-type hepatitis.

Because children are relatively young, their organs and immune systems have not yet fully matured; thus, their resistance to external pathogens is low, making them more susceptible to hepatitis B virus (HBV) infection than adults. So, what are the early symptoms of hepatitis B in children?

Other indicators include failure to pass stool (acholic stools), occasional diarrhea with pale, clay-colored stools, dark yellow or tea-colored urine, and yellow staining left by urine on diapers or clothing—these are all characteristic features of jaundice-type hepatitis.

Additionally, children infected with HBV may exhibit lethargy. A normally lively, active, and playful child who suddenly becomes fatigued, drowsy, or listless warrants close attention. Hepatomegaly may also occur, accompanied by dull pain or persistent distension in the right upper quadrant of the abdomen—particularly noticeable at night. In some cases, splenomegaly may develop, along with left upper quadrant (splenic) pain; affected children often unconsciously rub or press their abdomen with their hands.

Understanding the early symptoms of hepatitis B in children can aid timely recognition and prompt treatment.
