Introduction
Generally speaking, in Traditional Chinese Medicine (TCM), the heart “opens into” the tongue; thus, abnormalities of the tongue often reflect excessive heart fire—manifesting as oral ulcers, irritability, skin abscesses or boils, dry mouth, thirst, and constipation. Additionally, TCM holds that the heart and small intestine are interior-exterior paired organs; therefore, damp-heat in the small intestine is frequently associated with excessive heart fire, with frequent urination being the most typical symptom.

Excess Heart Fire is a relatively common TCM syndrome, and many people experience it in daily life. So, what are the symptoms of Excess Heart Fire?

Symptoms of Excess Heart Fire

In TCM theory, the Heart opens to the tongue; thus, manifestations on the tongue often reflect Heart Fire. Common signs include oral ulcers, infections, irritability, quick temper, skin sores or boils (especially on the face), dry mouth, excessive thirst, and constipation—all indicative of Excess Heart Fire. Additionally, according to TCM, the Heart and Small Intestine are interior-exterior paired organs; therefore, Damp-Heat in the Small Intestine is frequently associated with Excess Heart Fire. The most typical symptom is urinary frequency, urgency, and dysuria.

Thus, patients with urinary tract infections may find relief from Damp-Heat in the Small Intestine by clearing Heart Fire. A classic TCM formula for this purpose is Dao Chi San (Guiding Redness Powder), which clears Heart Fire to resolve Small Intestine Heat, thereby alleviating urinary frequency, urgency, and burning sensation during urination, and improving Damp-Heat symptoms in the Small Intestine. Moreover, if the condition arises from consuming spicy or stimulating foods, cultivating healthy dietary habits is essential—avoiding spicy, stimulating, and greasy foods, drinking plenty of water, and favoring light, bland meals.

If Excess Heart Fire results from emotional agitation or excessive mental stress, oral administration of herbal formulas such as Huang Lian Shang Qing Pian (Coptis Upper-Clearing Tablets), Niu Huang Qing Xin Wan (Bezoar Calming the Heart Pills), or Long Dan Xie Gan Wan (Gentiana Liver-Draining Pills) may be recommended to effectively clear Heart Fire. Patients should also manage their emotions carefully, limit intake of chili peppers, lamb, and alcohol, and minimize consumption of spicy and stimulating foods.
