Swelling can be reduced through surgery or medication. In the early stages of induration, if there is significant swelling, conservative treatment may be initiated first—such as local cleansing with warm water baths, followed by application of ichthammol ointment and appropriate use of anti-inflammatory medications—which can lead to gradual recovery. When the abscess enlarges and pain worsens, drug therapy is generally considered less effective. Surgical intervention should then be considered. Currently, the most fundamental treatment for perianal abscess is surgical incision and drainage.
