Some patients may develop dilated pupils after intracerebral hemorrhage, which is a sign of brain herniation. This usually occurs when excessive bleeding compresses the oculomotor nerve and brainstem. If a patient exhibits prolonged bilateral pupil dilation, it indicates a severe condition requiring immediate CT scanning and surgical craniotomy to remove the intracranial hematoma.
