Early symptoms of ovarian cysts include abdominal pain and bleeding. Because the cyst itself has a certain weight, during intestinal peristalsis or changes in body position, the cyst's stalk may be pulled, affecting the infundibulopelvic ligament in the pelvic cavity, leading to discomfort such as lower abdominal bloating and a sensation of downward pressure. These are early manifestations of ovarian cysts. Pain typically does not occur when the cyst remains intact. However, during an acute episode—such as when the cyst becomes twisted, ruptures, or becomes infected—abdominal pain often develops.
