In the initial stage of sacroiliitis, pain is the main symptom. In the early phase, it often occurs after physical activity. The pain is relatively insidious, presenting as a persistent dull ache that can be relieved by rest. As the condition progresses, joint mobility becomes restricted, and pain may occur even at rest or sometimes during sleep. Sacroiliitis can manifest with various types of pain, including low back pain, hip pain, proximal thigh pain, or groin pain. Patients commonly experience joint stiffness upon waking in the morning, which gradually improves with movement.
