Scrophularia (Xuan Shen) not only has the effect of clearing heat and detoxifying, but also helps resolve phlegm, eliminate dampness, moisten the lungs, and relieve cough. It is particularly effective in alleviating symptoms such as cough due to phlegm-heat and sore throat. In terms of herb combinations, Scrophularia is commonly used with Lu Gen (reed rhizome), Huang Qin (Scutellaria root), and others to enhance its heat-clearing and detoxifying actions. In severe conditions involving toxin invasion of the nutrient blood and blood level—such as high fever causing erratic blood circulation, loss of consciousness, and delirium—it can be combined with Xi Jiao (rhinoceros horn, now typically substituted), Sheng Di (raw Rehmannia root), and Dan Pi (Moutan bark) to clear heat, cool the blood, detoxify, and calm the spirit. For excessive stomach heat manifesting as dry mouth, thirst, and burning epigastric pain, Scrophularia may be used together with Pu Gong Ying (dandelion), Huang Lian (Coptis root), and Wu Zhu Yu (Evodia fruit) to clear heat, purge fire, harmonize the stomach, and relieve pain. Patients with hypertension or cerebral hemorrhage should avoid using Scrophularia, as long-term use may worsen their condition and even pose life-threatening risks.
